ADK@home
ADK@home is a concept where we aim to bring our services right to your home. This program is specially tailored to meet the needs of elderly patients above 65 years of age who may not need hospitalization, yet require routine medical interventions.
Given that the Maldives population is aging and the community resources are becoming less and less while the hospital beds cannot be allocated for such patients, it is of need to establish programs that cater for these specific populations.
The ADK@home program will make an assessment of those patients who wish to enroll and a suitable program among the three plans will be proposed for enrollment.
PLANS
Three levels of packages are designed to reflect the different patient profiles that will fall into the ADK@home program. The three packages are named as follows:
GREEN
People with a profile that requires the least monitoring and intervention will fall into this plan.
Initial assessment
Will include a consultation from the ADK@home team. Estimated to take about 40 to 50 minutes.
Review
Depending on the case, 1 to 2 reviews will be conducted each month. This will take about 20 to 30 minutes.
Investigations
Will be occasional based on need.
Team
ADK@home team consists of a doctor and a nurse.
Other
Referrals to other specialists or complex interventions will be based on need occasionally.
Costs arising from these referrals are excluded from this package.
YELLOW
People with a profile that require more frequent monitoring and intervention will fall into this plan.
Initial assessment
Will include a consultation from the ADK@home team. Estimated to take about 45 to 60 minutes.
Review
Depending on the case, 2 to 4 reviews will be conducted each month. This will take about 20 to 30 minutes.
Investigations
Some investigations will be prescribed 1 to 2 times monthly.
Team
ADK@home team consists of a doctor and a nurse.
Other
Referrals to other specialists or complex interventions will be based on need 2 to 4 monthly.
Costs arising from these referrals are excluded from this package.
RED
Patients with a profile that require the most frequent and more complex monitoring and intervention will fall into this plan.
Initial assessment
Will include a consultation from the ADK@home team. Estimated to take about 45 to 60 minutes.
Review
Depending on the case, 4 or more reviews will be conducted each month based on need. This will take about 20 to 30 minutes.
Investigations
Some investigations will be prescribed 4 to 5 times monthly.
Team
ADK@home team consists of a doctor and a nurse.
Other
Referrals to other specialists or complex interventions will be based on need 1 to 2 monthly.
Costs arising from these referrals are excluded from this package.
A person will be enrolled to a plan after the assessment and with agreement of that person. A written document of consent for treatments and payment will be signed to formally enroll a person in the packages.
PRICING
Pricing is based on an initial subscription and monthly payments for a period of one year. The subscription fee and the monthly payments are based on the complexity and load the enrolled person will take for their regular engagements with the ADK@home team.
